Thank you for your interest in scheduling an appointment with Twin Cities Veterinary Behavior Referral Service. Appointments are scheduled Monday through Friday with the earliest appointments at 8am and the latest at 4pm. Evening and weekend appointments are available for an additional fee. After reading this paragraph, please click the link below to fill out the history form(s) for your pet(s); please fill out one form for each pet that you wish to be evaluated. Once you submit the completed form, I will contact you to schedule your pet’s appointment.

When I come to your home to evaluate your pet’s behavior, the appointment will typically last from 2 to 2.5 hours. During that time I will ask additional questions about your pet, I will observe how your pet responds to a variety of situations and we will discuss the diagnosis, prognosis and treatment plan details. At the end of the appointment, you will have a treatment plan in place to address your pet’s behavior. Treatment plans typically consist of a combination of management strategies, behavior modification techniques and, in some cases, medication options. In some cases, it is not possible to provide a complete treatment plan during the initial assessment and additional appointments or follow-up may be necessary in order to fully address the problem.

After the appointment I will summarize my recommendations and provide you with a written treatment summary within approximately 1 week of your pet’s appointment. That summary is also sent to your pet’s regular veterinarian so that they are aware of the diagnosis and treatment recommendations. Each appointment includes three months of unlimited follow-up by phone and email so that we are able to stay in close contact and adjust the treatment plan as we observe your pet’s response. If additional treatment or communication is required, recheck appointments and communication period extensions are available.

The cost for a diagnostic appointment is $365.00 (dog) or $335.00 (cat) and includes evaluation of the history form(s), the house call consultation, a written summary and 3 months of follow-up. A travel fee of $0.95 per mile roundtrip from White Bear Lake, MN to / from your home will also be added to the cost of the appointment. If equipment, medical testing or medication is recommended, those charges will be in addition to the appointment and mileage fees. Recheck appointments are billed at a rate of $195.00 + mileage and also include an additional 3 months of unlimited follow-up. The communication period can be extended in 3 month increments for a fee of $95.00 up to one calendar year from your pet’s most recent house call appointment. I accept personal checks and credit cards (Visa, MasterCard, American Express and Discover) as forms of payment. Payment is due at the time of the appointment.  

Additional details:

• With your permission, I frequently use a digital audio recording device during appointments in order to keep track of important details that pertain to your pet’s diagnosis and treatment plan; please let me know if this is unacceptable to you for any reason (permission to record is not required in order to schedule an appointment).
   
• Once your pet’s appointment is schedule, please provide at least a 48 hour notice for any cancellations or reschedule requests to avoid a $100.00 late cancellation fee. The fee will not be assessed for reasons such as adverse weather conditions, personal illness, family emergency, etc.
